Does this lens come with mounting hardware or adhesive?

The lens assembly itself is included. Adhesive or fastener type for securing into your mirror housing is not confirmed — verify your current mirror housing setup before purchase to ensure compatibility with what you already have or what you're sourcing separately.

Will this fit if my mirror housing is cracked or damaged?

This is a glass lens replacement only. If your mirror housing is compromised, the lens will sit loose or misaligned. Inspect your housing for cracks, warping, or damage before ordering.

Can I order just one side or do I need to buy both?

This is a right-side lens only. If you need both mirrors replaced, you'll order this one for the passenger side and a left-side lens separately for the driver side.
